2) Proof of Departure: Copy of airline ticket or travel itinerary or original letter
    from your company (For sample letter click here ).
3) Application fee: $60 (payable by check or money order made out to Department of State).
4) Courier fee:   5-10 day service         $110.00
                         1-4   day service         $195.00
5) Form DS-4085     On line Additional Visa Pages Application

                                 THIS FORM MUST BE COMPLETED & CREATED ONLINE.
Above courier fee includes all shipping charges and is payable by cash, credit card or check
made out to Passport Express.
