Answer all
questions--especially
"Contact in Brazil" section.
b. Print
the cover sheet and sign it
inside the signature box
exactly the same as your
passport is signed.
3. Copy of your
travel itinerary or airline
tickets

One passport photo (can be
taken at our office if you
come in person)
5.
Application fee:

Proof of residence (photocopy
of driver's license or
utility bill i.e. water, gas,
electric)

the address on proof of
residence.
Above fees include photos
and all shipping charges and are payable by cash, check, or
credit card.

card or visa in U.S. or
original I-94.
*Minors
