| 2) Birth Certificate (certified copy) OR Previous US Passport OR Naturalization Certificate. |

| 3) Proof of Departure: Copy of airline ticket or travel itinerary or original letter from your company (For sample letter click here). |

| 4) Application fee: $60.00 (payable by check or money order made out to Department of State). | |||
| 5) Letter of explainations of why passport was limited. | |||
| 6) Courier fee: 5-10 day service $110.00 1-4 day service $195.00 |
