Lost/Stolen Passport

Please print and and follow instructions carefully


Children under the age of 16:  Must be accompanied by both parents at the post office or at the courthouse to sign the application. Parents must submit identification and evidence of relationship to the child (ie child's certified birth certificate).  There are exceptions to this requirement, please call us for detailed instructions.

For children 16-17 years of age: It is preferred to have at least one parent accompany them to co-sign the application at the post office or the courthouse.

Adults 18 years and older:  Can sign for themselves.

Please bring the following items to our office at 1107 Rio Grande St. before 3pm so you may meet the 4pm deadline at the post office or the courthouse. Directions to Passport Express. 

  1. Old Passport OR Birth Certificate (certified copy), issued by the State, City, or County bearing the offical seal: HOSPITAL COPY NOT ACCEPTED, OR Naturalization Certificate. 
  2. Valid Photo Driver's License, (If your Driver's License is newer than 6 months bring supplemental documents.  For recommended list click here). 
  3. Proof of departure: Copy of airline ticket or travel itinerary or original letter from your company.  (For sample letter click here).
  4. Application Fee: $135.00 if you are 16 or older $120.00 if you are younger (payable by check or money order made out to the Department of State) & execution fee: $25.00 payable by cash or credit card. 
  5. Courier fee:             5-10 day service      $105.00
                                   1-4   day service      $185.00
  6. Application Form(s):

          Form DS-11:       On line Passport Application Form, filled out by all applicants.
                                                 PLEASE COMPLETE FORM, CREATE FORM,  PRINT FORM.
          Form DS-64:      Lost or Stolen Passport Application, if you have had a valid 
                                     passport lost or stolen.

   Form DS-3053:   Notarized Statement of Consent, filled out and signed by the non-applying parent of minors under the age of 16.

Above courier fees include photos and all shipping charges and is payable by cash, credit card, or check made out to Passport Express.

Above service days indicates processing time at the passport agency.
