NAME CHANGE  PASSPORT RE-APPLICATION

Please bring the following items to our office at 1107 Rio Grande during our
office hours: Mon - Fri  9 - 5:30.  Directions to Passport Express

1)  Passport (issued within the last one year).
2)  Proof of Departure: Copy of airline ticket or travel itinerary or original letter from your
      company, (For sample letter click here ).
3)  Certified copy of the name change (marriage license, divorce decree, etc).
4)  Application fee: $60.00 (payable by check or money order made out to Department of State).
5)  Courier fee:   5-10 day service         $110.00
                          1-4   day service         $195.00
6)  Form DS-5504     On line Passport Re-Application.

                                  THIS FORM MUST BE COMPLETED & CREATED ONLINE.
Above courier fee includes photos & all shipping charges and is payable by cash, credit card or check
made out to Passport Express.
