   B. I was 16 years or older when my most recent U.S. passport was issued.
Yes No
   C. My most recent U.S. passport was issued within last 15 years.
Yes No
If the answer is "No" to any of the above questions, you will not qualify for passport renewal, please refer to  New Passport Instructions.

If the answer is "Yes" to all above questions, please continue.

Please bring the following items to our office at 1107 Rio Grande during our
office hours: Mon - Fri  9 - 5:30.  Directions to Passport Express


1)  Most Recent U.S. Passport: 

2)  Proof of name change if any: (Certified marriage license, divorce decree, etc.)

3)  Proof of Departure: Copy of airline ticket or travel itinerary or original letter from your
      company (For sample letter click here ).
4)  Application fee: $135.00 (payable by check or money order made out to Department of State).
5)  Courier fee:    5-10 day service         $105.00
                           1-4   day service         $185.00
6)  Form DS-82     On line Passport Renewal Application.

                              PLEASE COMPLETE FORM, CREATE FORM,  PRINT FORM.

Above courier fee includes photos & all shipping charges and is payable by cash, credit card or check
made out to Passport Express.
