DO I QUALIFY FOR PASSPORT RENEWAL?

If the answer is "No" to any of the above questions, you will not qualify for passport renewal, please refer to  New Passport Instructions.

If the answer is "Yes" to all above questions, please continue.

DOCUMENTS YOU NEED TO SEND US:

1)  Most Recent U.S. Passport: 

2)  Proof of name change if any: (Certified marriage license, divorce decree, etc.)

3)  Two identical passport photos taken within the last three month.

4)  Proof of Departure: Copy of airline ticket or travel itinerary or original letter from your
      company (For sample letter click here ).

5) 2 original Authorization Forms , click here to download (please print 2 copies, complete, sign & date)

6)  Application fee: $170.00 (payable by check or money order made out to Department of State).

7)  Form DS-82     On line Passport Renewal Application.

8)  Courier fee:       5-10 day service         $135.00
                                1-4   day service         $210.00

Above service days indicate processing time at the passport agency.

 Please bring by or overnight the above documents & fees to the following address:

State Department regulations state that the application has to be submitted within 4-5 days of when it is dated. Please date your application for the day Passport Express will receive your materials.

Please call us or email us with your tracking number and which shipping company you are using so that we may be able to monitor your package. Your passport application will be hand carried to the passport office. When the passport is ready, it will be sent to the address on the “Return Shipping” form below. Please note a street address (not PO Box) and a phone number where you can be reached must be included.

RETURN SHIPPING

Please note: After your documents are processed, they will be shipped to you via FedEx. Please provide an address where someone will be available to sign for the package. FedEx will not deliver to P.O. Boxes. If you provide a residential address and no one is available to sign for your package, FedEx may leave your package at the door.
