SUPPLEMENTAL IDENTIFICATION LIST

If your driver's license is newer than six months you are requested to submit copies of several documents in order to better establish your identity.
The following is a suggested list of documents, which have proved helpful in processing other passport applications.

1. Expired driver's license or expired state identification card
2. High school yearbook
3. High school transcripts/ Diploma
4. Expired passport
5. Military records
6. Professional license (nurse, beautician, pilot, mechanic, etc)
7. Baptismal certificate/ Confirmation and communion certificate
8. Voter registration card
9. Income tax records and returns/ W-2 forms
10. Banking records/ statements
11. Social security card
12. Mortgage, lease, or rental agreements/ contracts
13. Work ID cards and records
14. Marriage certificates and licenses/ Divorce decrees
15. Medical and hospital records
16. Union membership card
17. College diploma and/or transcript
18. Technical trade school diploma and/or transcript.
19. Insurance papers and ID cards/ Tax assessments
20. Vehicle registration and/or title
21. Car insurance papers and ID cards
22. Library card
23. Student ID card
24. Selective service registration/ classification
25. Others
Submit at least 10 documents that are five years or older.

Please note: It is also recommended that at the time of the execution of the application,
the applicant be accompanied by a witness who will fill out an Affidavit
of Witness (form DS-71). Witness must be a US citizen or US permanent resident
& has known the applicant for at least 2 years & present proper identification.
